    2. Click on the Service Catalogs link and select the Human Resources tile.

    3. Click on Annual Open Enrollment.

    4. Read your Welcome letter for instructions. Click continue then go through each tab to make any necessary changes. Read the questions and pop up instructions on each tab carefully.

    5. The final tab is the USU Enrollment Review. Once you confirm the information is accurate, click Submit. A confirmation email will be sent to your preferred email address. Please retain the confirmation email until after August 1st. Utilize Banner Access to verify your deductions reflect your open enrollment elections.

Two Medical Networks

The network options are:

Provides access to 40 Utah hospitals including Logan Regional Hospital, Cache Valley Specialty Hospital, University of Utah Hospital, Huntsman Cancer Hospital and Primary Children’s Hospital.

This is the broadest network offered. It currently provides access to 49 Utah hospitals including Logan Regional Hospital, Cache Valley Specialty Hospital, University of Utah Hospital, Huntsman Cancer Hospital, Primary Children’s Hospital and McKay-Dee Hospital.

Regence Blue Cross Blue Shield Pharmacy-All Medical Plans

Regence Blue Cross Blue Shield is now the Pharmacy Benefit Manager (PBM) for all USU medical plans (Blue, White and HDHP).  Managing prescriptions can be difficult and Regence is available to assist you with all your prescription needs.  Here are five tips for getting the most from your new prescription benefits plan:

 

  1. Check the drug list.

When it comes to choosing medications, it’s important to know how the list of covered drugs—or formulary—works. The covered-drug list divides medications into multiple tiers, each with its own cost share. Before a medication is added to the list, Regence’s team of doctors and pharmacists carefully evaluate how safe and effective it is while assessing whether it will improve health. What does this mean for you? By knowing whether your benefit covers your medication as well as which tier it falls under, you’ll know how much you owe.

Register and sign in to regence.com to search the list of covered drugs. You’ll need your new member ID card to register. Select Pharmacy benefits and Find a drug. If you don’t have your member ID card yet, visit regence.com and select Members > Pharmacy benefits, select your insurance coverage (51+ employees), then select Find a drug. Enter any drug name in the search field to find out if it's covered. If you don’t see your prescription drug on the list, you could pay more out of pocket the next time you get it filled.

Generic and brand-name medications have the same strength, quality and purity. But, generics can cost up to 80% less. So, ask your doctor if there is a generic drug that will work for you.

To save money, talk to your doctor about switching to a generic equivalent on our list of covered drugs.

  1. Check to see if your pharmacy is in-network.

We have participating pharmacies across the U.S. Follow the instructions above for checking the drug list and select Find a pharmacy. Be sure to show your new member ID card to the pharmacist to make sure your claims get processed accurately.

3. Use Express Scripts Pharmacy for the best home delivery prices.

  1. Use Accredo for specialty drugs.

  1. Check to see if your prescription drug requires pre-authorization.

Some prescription drugs require pre-authorization before Regence will cover them to make sure you’re getting the most effective, highest value prescription drug available. You can save time and money by checking before you go to the pharmacy. In many cases, brand name drugs can be replaced with equivalent generic alternatives at a much lower cost. Understanding pre-authorization

To ensure you’re getting an effective drug at an affordable price, we review prescriptions for some medications before we cover them. Drugs on the pre-authorization list include many for which equal or more effective and lower-cost options exist.

If your drug needs pre-authorization, you’ll want to do one of two things:

  1. Talk with your doctor to see if there’s an alternative treatment that does not require pre-authorization.

OR

  1. Have your doctor or pharmacist request pre-authorization for your medication. You may need to get that authorization before you can get your prescription filled.

Dental insurance is provided by Regence BCBS of Utah

  • Examinations, fillings, X-rays, sealants, etc., covered at 80%.
  • Sealants are covered for dependent children only.
  • Maximum benefit per person per contract year is $1,500.
  • Prosthetics: Dentures, bridges, crowns, implants, etc., covered at 50%.
  • Orthodontics: Eligible benefits are paid at 50%.
  • Maximum benefit on orthodontics is $1,500 per person per lifetime.

Dual Coverage Insurance

When both spouses or domestic partners are employed at Utah State University in benefit eligible positions and wish to be part of the Dual Coverage medical/dental benefit, they may enroll in Dual Coverage within 30 days of beginning a new job, within 30 days of a life event, or during Open Enrollment.

Each employee will be enrolled in the same medical plan and each employee will pay a medical and dental premium. The employee that is the primary subscriber will pay the family or couple premium in the appropriate salary tier. The other employee will pay single premium for medical and dental according to their appropriate salary tier. We are required to set up the primary subscriber as the employee earning the lesser salary, and the other employee will be listed as their dependent. We review salaries once a year in July to determine if any primary subscriber changes should be made

Domestic Partner Tax

When you elect to cover a domestic partner on health insurance there is an extra tax you are subject to. If Dual Coverage Insurance is elected with a domestic partner, both employees will be subject to the domestic partner health insurance tax. Contact HR for details: 435-797-0216

Flexible Spending Account (FSA) - Is it Right for You?

*Flexible spending does not rollover year to year so you must re-enroll every year*

The FSA is available for those who enroll in the Wellness (White) and High Premium (Blue) plans. If you enroll in the HDHP plan you do not qualify for the Medical FSA, but you may contribute to the Dependent Care FSA.

Utah State University offers employees a Flexible Spending Account (FSA) for both health expenses and dependent care expenses that are incurred during the plan year. Employees allocate a portion of monthly earnings, pre-tax, that can be used for health or dependent care expenses, which can result in substantial tax savings to employees who participate in the FSA.

The maximum you may set aside for health expenses is $2,750 per year. Dependent care maximum amount is $5,000. To participate, you must sign up ANNUALLY online during open enrollment. Remember that the money you set aside in your flexible spending account must be used within the same plan year.

MEDICAL AND DEPENDENT CARE FLEX SPENDING : Expenses may be incurred from July 1, 2021 through September 15, 2022. Medical and dependent care receipts must be submitted for reimbursement by December 15.

  • If you currently have an FSA and are moving to the HSA for next plan year, all FSA funds must be used by June 30th or you cannot contribute until October 1.
  • If you elect the HDHP and enroll in an HSA, you are not eligible for the medical flex spending account, but you may sign up for the dependent care account.

Health Savings Account (HSA)

Previous Contributions Continue - You May Adjust Contributions at Any Time

The High Deductible Health Plan (HDHP) encourages wellness and prevention, wise spending, and health-related savings. The HSA is available in conjunction with the HDHP.

An HSA is a triple-tax-advantaged account with tax-free contributions, account earnings, and qualified withdrawals are tax-free. It is an individual account specifically for qualified medical expenses. USU partners with HealthEquity, an industry leader in HSA administration.  

The money you contribute into your HSA is in an account controlled by you and it earns interest. It allows you to save as your budget allows so you’re prepared when those expenses come up.

HSA money belongs to you if you change medical plans, take another job, or retire. You can make tax free contributions in 2021 to your HSA up to $3,600/single or $7,200/family. This money continues to roll over each year.

Flexible Spending

  • If you had a flexible spending account last year with ASI, you will need to finish out that account. Be sure not to leave any money on the table.
  • If you had flex last year, and want it again this year, sign up with WEX and get familiar with how to use it.
  • If this is your first time with a flexible spending account, this should not affect you as much. Get familiar with how to use flex with WEX, research, and ask WEX any questions that come up.

Health Savings Account

  • Do not worry, all of the money you had with Health Equity is still yours.
  • All contributions starting 7/1/2021 will go to WEX. Get familiar with how to use an HSA with WEX.
  • If you want to move your HSA money from Health Equity to WEX, more info will come on that.


The Hartford - Life & Disability Insurances

SUPPLEMENTAL LIFE INSURANCE: The open enrollment period is the time to purchase additional Supplemental Life Insurance for yourself or your dependents.

Life Insurance Rates and Premiums

SHORT-TERM DISABILITY INSURANCE: If you have not already purchased Short-term Disability Insurance, you have the opportunity to apply for it during open enrollment. This is an inexpensive way to protect your income if you experience a short-term disability. During this annual enrollment period you can elect STD coverage which is guaranteed issue- it is available without having to provide information about your health.

The Hartford Portal: During open enrollment, you will be able to access The Hartford portal to review life and STD benefit elections for you and your covered dependents; add supplemental life insurances for you, your spouse, and/or children; add short-term disability benefit if you do not already have it; and confirm, add, or update your beneficiary information.

Beneficiary Information: Open enrollment is a good time to check beneficiary information on your life insurance policies and other benefits.
