ACTION REQUIRED REGARDING MEDICAL BENEFITS

Utah State University works diligently to provide you with the best possible medical insurance options at the lowest cost. Our current network offered through Regence BlueCross BlueShield (BCBS) of Utah is known as the Preferred Blue Options (PBO) network. Due to circumstances beyond the control of USU and Regence BCBS, the PBO network will no longer be available after June 30, 2015. As a result of this change, USU and Regence BCBS will offer 2 new insurance network options to benefit eligible employees for the 2015-16 plan year.

All benefit eligible employees must complete a new medical enrollment form and select a network.

BENEFITS OPEN HOUSE - May 27-28, June 3, 2015, 9:00 a.m. - 4:00 p.m. Center and West Colony Rooms, TSC

The Office of Human Resources is offering Open Enrollment and Benefits Open Houses on May 27-28 and June 3, 2015, from 9:00 a.m. - 4:00 p.m. in the Center Colony and West Colony rooms of the Taggart Student Center. Human Resources representatives will be available to assist you and answer questions relating to benefits and open enrollment.

Two Medical Networks

The network options are:

Provides access to 40 Utah hospitals including Logan Regional Hospital, Cache Valley Specialty Hospital, University of Utah Hospital, Huntsman Cancer Hospital and Primary Children’s Hospital.

This is the broadest network offered. It currently provides access to 49 Utah hospitals including Logan Regional Hospital, Cache Valley Specialty Hospital, University of Utah Hospital, Huntsman Cancer Hospital, Primary Children’s Hospital and McKay-Dee Hospital

Health Insurance

USU offers 3 medical insurance plans: Choice High Deductible Health Plan (HDHP), Wellness (White), and High Premium (Blue). The Choice HDHP is a non-traditional plan with the option of a Health Savings Account. With the Wellness (White) and High Premium (Blue) plans you have the option to also enroll in Flex Spending. All 3 plans include Vision Care.

VRx — USU’s Pharmacy Benefit Manager

Beginning July 1, 2015, VRx Pharmacy Services will become Utah State University’s new pharmacy benefit manager for employees participating in the High Premium Plan and the Wellness Plan. Regence BCBS will continue to provide pharmacy benefits for employees on the Choice High Deductible Plan. VRx is a Salt Lake City, Utah-based Pharmacy Services Manager dedicated to improving pharmacy services while placing significant emphasis on providing quality customer service. Dental insurance is provided by Regence BCBS of Utah

  • Examinations, fillings, X-rays, sealants, etc., covered at 80%.
  • Sealants are covered for dependent children only.
  • Maximum benefit per person per contract year is $1,500.
  • Prosthetics: Dentures, bridges, crowns, implants, etc., covered at 50%.
  • Orthodontics: Eligible benefits are paid at 50%.
  • Maximum benefit on orthodontics is $1,500 per person per lifetime

Flexible Spending Account (FSA) - Is it Right for You?

*You Must Enroll Every Year*

The FSA is available for those who enroll in the Wellness (White) and High Premium (Blue) plans. If you enroll in the Choice (HDHP) plan you do not qualify for the Medical FSA, but you may contribute to the Dependent Care FSA.

Utah State University offers employees a Flexible Spending Account (FSA) for both health expenses and dependent care expenses that are incurred during the plan year. Employees allocate a portion of monthly earnings, pre-tax, that can be used for health or dependent care expenses, which can result in substantial tax savings to employees who participate in the FSA.

The maximum you may set aside for health expenses is $2,550 per year. Dependent care maximum amount is $5,000. To participate, you must sign up ANNUALLY online during open enrollment. To enroll go to enroll.asiflex.com and use the employer code USU. Remember that the money you set aside in your flexible spending account must be used within the same plan year.

MEDICAL AND DEPENDENT CARE FLEX SPENDING : Expenses may be incurred from July 1, 2015 through September 15, 2016. Medical and dependent care receipts must be submitted for reimbursement by December 15, 2016.

  • FlexMinder: FlexMinder can monitor your health plan expenses through Regence BCBS to identify claims with qualified out-of-pocket health care expenses that can be reimbursed from your health care FSA. You can then direct FlexMinder to prepare and submit FSA claims for the identified claim amounts. This can be done as frequently as you wish. You can also allow FlexMinder to “shoe-box” the identified claims throughout the year and then submit only once at the end of the plan year. Health Savings Account (HSA)

Previous Contributions Continue - You May Adjust Contributions at Any Time

The Choice High Deductible Health Plan (HDHP) encourages wellness and prevention, wise spending, and health-related savings. The HSA is available in conjunction with the HDHP.

An H.S.A. is a triple-tax-advantaged account where your contributions, account earnings, and qualified withdrawals are tax-free. It is an individual account specifically for qualified medical expenses.. USU partners with HealthEquity, an industry leader in H.S.A. administration.  Visit Health Equity to learn more about them.

The money you contribute into your H.S.A. is in an account controlled by you and it earns interest. It allows you to save as your budget allows so you’re prepared when those expenses come up.

H.S.A. money belongs to you if you change medical plans, take another job, or retire. You can make tax free contributions in 2015 to your H.S.A. up to $3,350/single or $6,650/family. This money continues to roll over each year.

Note: if you have “other coverage” such as coverage on an HSA qualified HDHP you are not eligible to both contribute to an H.S.A. due to tax implications.

The Hartford - Life & Disability Insurances

SUPPLEMENTAL LIFE INSURANCE: The open enrollment period is the time to purchase additional Supplemental Life Insurance for yourself or your dependents. If you are interested in Supplemental Life Insurance, you may purchase it from The Hartford Insurance Company.

SHORT-TERM DISABILITY INSURANCE: If you have not already purchased Short-term Disability Insurance, you have the opportunity to apply for it during open enrollment. This is a very inexpensive way to protect your income if you experience a short-term disability.

During open enrollment, The Hartford will send a letter to your home explaining the premiums and coverage you have and how to make changes online or request Supplemental Life Insurance or Short-term Disability Insurance.

You may be required to provide evidence of insurability for Supplemental Life Insurance and Short-term Disability Insurance which may include a health questionnaire or exam. The Hartford will communicate directly with you regarding additional information required once they receive your request. Open enrollment is a good time to check beneficiary information on your life insurance policies.

Zurich - Accidental Death and Dismemberment Insurance

During open enrollment, you may purchase Supplemental Accidental Death and Dismemberment (AD&D) Insurance from The Zurich Company. If you purchase AD&D insurance from The Zurich Company you will also have access to travel assistance insurance that offers medical, legal, and personal assistance when you are 10 or more miles from home.

MetLaw Prepaid Group Legal / Hyatt Legal Plan

With MetLaw, the group legal plan made available by Hyatt Legal Plans, Inc., a MetLife company, you have access to a network of attorneys on retainer for a low monthly premium paid via payroll deduction.  The monthly premium is $21.25.  You will have access to legal services and representation from local attorneys on a wide range of matters including wills and estate planning, financial matters, real estate, traffic offenses (DUI not included) and more.  There are no co-pays or deductibles.  For example, you can have a local attorney prepare full estate planning documents for you and your spouse for just $21.25 per month.

Once enrolled, you will be required to remain in the plan for the full benefit plan year.  If you are already enrolled in the plan your coverage will automatically renew unless you call during open enrollment to cancel.
